It was the investigators of the 24-page preliminary report, published Tuesday by the National Transportation Safety Board (NTSB), then reported by CNN, who came to this conclusion.

About 10 hours before the incident, the boat experienced two power interruptions, one of which was caused by crew error, according to the report.

At the time of the failure, the vessel was under the control of a trainee pilot, who was accompanied by a more experienced driver.

When the pilots boarded, the captain said the boat was in good working order, according to the documents.

NTSB investigators confirmed that crew members tested negative for drugs and alcohol, and the fuel tested negative three times for contaminants.
